ATTENTION ALL PRIDE BOOSTER GROUP TREASURERS AND PRESIDENTS......

Your 2009/2010 group financial information is now due in order for the MPHS Pride Boosters, Inc. to prepare their 2010 IRS Form 990.

Please submit the following on or before November 1, 2010 to fvanalstine@cox.net:

1. A copy of your 6/30/09 bank statement - you are welcome to hide your bank account information.
2. A copy of your 6/30/10 bank statement.
3. A copy of your internal financial statements or whatever report you provide to your group.
4. Please complete the following spreadsheet to the best of your ability-Click on this for your link.

MPHS Pride Boosters Donation Receipt Instructions

 Per the IRS rules a charitable organization is required to provide a written disclosure to a donor who receives goods or services in exchange for a single payment over $75.  A donor is responsible for obtaining a written acknowledgement from a charity for any single contribution of $250 or more before they can claim a charitable contribution. 

 Based on these rules, the MPHS Pride Boosters require that all Pride Groups use the following forms to ensure compliance:

• Give receipts for donations over $75 using the Donation Receipt Form - Short Version.  See attached donation receipt over $75.
• Give receipts for donations over $250 using the 3-part Donation Receipt Form.  See attached donation receipt over $250.  Your group President or Treasurer can obtain these forms by emailing fvanalstine@cox.net or mphsprideboosters.com.  Remember that the donor, your group and the MPHS Pride Boosters must all receive a copy of the completed 3-part form in order for your donor to be able to claim a charitable contribution of their donation.
